I-140 application was sent to CIS on 19th Sep, 2012....CIS receipt date 24th Sep, 2012.

I checked i-907instr located @ http://www.uscis.gov/files/form/i-907instr.pdf and it says 15 days max for processing.

"Premium Processing Service guarantees 15-calendar-day processing of certain employment-based petitions or applications. U.S. Citizenship and Immigration Services (USCIS) will refund the Premium Processing Service fee but continue to process the case unless, within 15 calendar days of receipt of a properly filed Form I-907, USCIS issues an approval notice or, where appropriate, issues a notice of intent to deny a request for evidence, or opens an investigation for suspected fraud or misrepresentation on the related petition or application.
The processing time has been met if an intent to deny or request for evidence is issued within 15 calendar days of receipt or if the case is referred for investigation of suspected fraud or misrepresentation. Once USCIS receives a response to the notice of intent to deny or request for evidence, USCIS will guarantee 15-calendar-day processing from the date the response was received. "

For i140 PP: CTS will file i140 PP if you are within 12 months of reaching your I-94 expiration date AND within 24 months of reaching your visa max out date. There is no additional fee from your end.

Submit your ETA9089 and check ...GIT will response within 4 weeks

try to get affidavit ( GIT will provide format ) from your previous Manager or Project team member. It will cost < INR 200
If your Manager is in US then get Notary (you can get from bank).

Yes you are right. You have to be in US for i-485. You can always go back after i-140 approve.

Unfortunately Current date for i-485 is unavailable

Point 3. You will have to remain offshore until the 365 day requirement has been met. We will file an H-1B for consular notification (not subject to the H-1B cap) for you while you are out of the country and 365 days are past your PERM filed date

After max-out If you come back to India then You must need approved i140 in order to get h1b extn from India.
