I have traveled almost every 3 weeks on dependent visa, after maxing out on my H1. I don't see any issue in traveling on B1/B2, but the chances of visa rejection might be little higher.

Just don't worry! You don't need to take FIOA route. Send a note to your SPOC and you will get the approval copy in a day or two.

I am not sure why SPOC is giving you a push back. I have just fwded UH's approval email to SPOC. Anyways I believe it is only taking 3-4 months in Texas SC, so its not a bad wait.
For your second question - I got the receipt number from SPOC and they will also share an approval copy as soon as they receive it.

SPOC requires an email from UH. A simple 'ok' or 'approved' email from UH will do. There is really no particular process for PP. Work with your unit chain of command (DM/EM/PgmM) to reach out to UH for approval.

Receipt - 3-4 business days.
Decision (Approval/RFE) - Regular 3-4 months (you can check trackitt tracker to get an idea) and 10-15 days for Premium.

Rocky - Do you have any recapture time left? If yes, then an extension can be filed with recapture time + 1 year based on pending perm, otherwise the answer is NO. My PERM was filed on Dec 19th 2012 and H1 extension was filed on Dec 20th 2013, so there was absolutely no delay from the immigration team. You can also get an approval from UH for Premium.
